The plumber starts by working out whether the tank has failed or something bolted to it has, because those lead to completely different conversations. Water on a basement floor can come from a loose connection, from a drain valve that won't seat, from the relief valve doing its job because pressure is too high, or from the tank shell itself rusting through. Only the last one is terminal. The connections get checked, along with the thermostats and the relief valve, and the anode rod gets pulled on a tank old enough to make that worth knowing. Pressure gets a look too, since a lot of homes on municipal supply here run higher street pressure than the plumbing wants, and without a working expansion tank that shows up as a relief valve weeping. Sediment is the other constant. Hard water leaves mineral on the tank floor, and that layer traps heat against the steel, which is what makes an old heater rumble. Basements are also where venting has to be right, so on gas equipment the flue and the combustion air get checked as part of any serious repair.

Sometimes, but often it's doing exactly what it was built to do because pressure in the system is too high. A failed or missing expansion tank, or high incoming street pressure with no regulator, will both cause it. The valve is the messenger rather than the fault.
There's no single number, since water hardness, incoming pressure and the state of the anode rod all move it. What's more useful than age alone is the condition: rust in the hot water, rumbling, or any weeping from the body all say the tank is near the end.
On one that's been flushed along the way, yes, since it removes loose sediment before it packs. On a tank that's gone many years untouched, the deposit is usually hardened into a crust that won't leave through the drain valve, and disturbing it can plug the valve open.
Sometimes, though it depends on gas supply, water lines, and above all on venting, since a gas heater has to have a proper path for exhaust. In older homes with masonry chimneys, that's usually the constraint that decides where the unit can sit.
